This practice is not contracted with any insurance panels.  Cash, checks, debit, HSA,  and credit cards are accepted.

Cancellation Policy

If you are unable to attend an appointment, please notify the office no later than 24 hours before the appointment. Otherwise, a cancellation fee will be charged.

Office hours: Monday-Thursday 9:00am-4:00pm. During office hours, the response time to your email or call is within a 24 hour period.  Calls and emails that are received outside of office hours will be returned on the following Monday.
